Tesamorelin is a synthetic analog of human growth hormone-releasing hormone (hGHRH) consisting of the full 44-amino acid sequence of GHRH with a trans-3-hexenoic acid moiety conjugated to the N-terminus. This modification improves stability against dipeptidyl peptidase IV (DPP-IV) cleavage at the His-Ala N-terminal bond, significantly extending the compound’s half-life in plasma versus unmodified GHRH in pharmacokinetic studies.
